How do I make an application to the School Ethics Committee?

You need to follow the steps given below:

Start by reviewing the risk table carefully. If your research falls into the “No risk” category you can apply for a waiver. In all other cases you must submit an ethics application form.
Before you can submit either application form to the School Committee, you need to provide an overview of your application. You will only be able to access this Google form while you are logged into your Wits Google account.

After clicking the form’s Submit button, a link will be displayed allowing you to enrol in the School Committee’s Ulwazi site.

Here, you will be able to submit your application form and all the associated documentation. If you are undertaking the fourth year investigation project, then only one student in the group must provide the overview and enrol in the Ulwazi site and they will make the application on behalf of the group.

Review the relevant sections of the guidelines for applicants so that you are able to make a complete and correct application. Prepare a zip archive including all the necessary documentation for the application that you are making (see the table below).

Bear in mind that the School Committee deadlines are fixed and that if you miss a deadline you will only be able to submit for the following month.
